NOTICE TO PATIENTS

Under federal law, healthcare providers are required to provide individuals who do not have insurance or who choose not to use their insurance with a Good Faith Estimate of expected healthcare costs before services are provided.

You have the right to receive a Good Faith Estimate explaining how much your medical care may cost.

This notice is provided in accordance with the federal No Surprises Act.

What is a Good Faith Estimate?

A Good Faith Estimate is an estimate of the expected charges for healthcare services, including:

  • Medical evaluations

  • Telehealth consultations

  • Follow-up visits

  • Chronic disease management visits

  • Medication management visits

  • Wellness consultations

  • Other healthcare services provided by Redefined Health & Wellness LLC

The estimate is based on information known at the time it is created and does not guarantee final charges.

Actual costs may vary depending on your medical condition, treatment needs, testing requirements, referrals, or additional services requested.

Who Is Eligible?

You are entitled to receive a Good Faith Estimate if you:

  • Do not have health insurance, or

  • Choose not to use your health insurance for services provided.

Your Rights

You have the right to:

  • Request a Good Faith Estimate before scheduling a service.

  • Receive a Good Faith Estimate in writing.

  • Ask questions about expected charges before receiving care.

  • Save a copy of your estimate for your records.

If Your Final Bill Is Higher Than Expected

If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charges through the federal patient-provider dispute resolution process.

Additional services, testing, referrals, or medications prescribed by outside providers, pharmacies, laboratories, imaging centers, specialists, hospitals, or other healthcare facilities are not included in these estimates.
